Anaemia in children

Anaemia in children is a condition where the amount of haemoglobin in the blood is lower than normal or the number of red blood cells (RBCs) are lower. Anaemia reduces the blood’s ability to carry oxygen to the body’s tissues. It is most commonly due to iron deficiency, poor nutrition, or blood loss. Anaemic children have pale skin, weakness, poor appetite, and delayed growth or development. Children should be given a nutritious diet rich in iron such as green leafy vegetables, meat, eggs.

What causes anaemia in children?

Iron deficiency is the most common cause which occurs due to insufficient iron intake from diet or poor absorption of iron. When children are in rapid growth periods the body may not produce as per the requirement.
Poor nutrition means lack of foods rich in iron, folic acid, and vitamin B12. Iron is the key content to make healthy red blood cells.
Blood loss could occur in children due to intestinal worms, injuries, surgery, or heavy menstrual bleeding in older girls.
Chronic infections or diseases like malaria, tuberculosis, or kidney disease can reduce red blood cell production.
Genetic disorders such as thalassemia or sickle cell anaemia, where the body produces abnormal red blood cells.
During infancy and adolescence, the children’s bodies need more iron to support increased blood production.

How is Anaemia diagnosed in children?

Medical history and physical examination:

  • History is collected regarding symptoms like fatigue, poor appetite, or pale skin.
  •
  • The doctor will do a physical exam to check the pale skin, lips, or eyes and delayed growth.

Blood tests:

  • Complete Blood Count test measures haemoglobin levels, red blood cell (RBC) count, and size of RBCs to confirm anaemia.
  •
  • Peripheral blood smear examines the shape and size of red blood cells to identify the type of anaemia.
  •
  • Serum ferritin and iron studies check the iron levels in the body to detect iron deficiency anaemia.
  •
  • Vitamin B12 and folate tests identify deficiencies causing anaemia.
  •
  • Reticulocyte count shows how well the bone marrow is producing new red blood cells.

Special tests (rarely done):

  • Hemoglobin electrophoresis detects genetic causes like thalassemia or sickle cell anaemia.
  •
  • Stool examination detects intestinal worms or blood loss.

Treatment of Anaemia in children

The treatment is based on the cause and aims to restore normal haemoglobin levels and improve overall health.

1. Iron supplementation

  • Iron supplements either in the form of syrup or tablets are prescribed if the anaemia is due to iron deficiency.
  •
  • Treatment must continue for 2–3 months after haemoglobin returns to normal to build iron deposit.

2. Dietary changes

  • Green leafy vegetables such as spinach, amaranth.
  •
  • Include meat, liver, eggs, and fish in the diet.
  •
  • Pulses, beans, jaggery, and fortified cereals
  •
  • Include vitamin C–rich foods like oranges, tomatoes, and guava to help absorb iron better.
  •
  • Ensure adequate intake of folic acid and vitamin B12 found in milk, eggs, and leafy greens.

3. Treatment of underlying medical causes

  • If intestinal worms are causing blood loss then deworming medicines are given,
  •
  • If anaemia is due to malaria antimalarial treatment is given.

4. Blood transfusion:

  • In severe anaemia or genetic blood disorders like thalassemia, under medical supervision blood transfusion is done.

5. Education:

  • Parents should be educated about a balanced diet, regular health check-ups, and completing iron therapy.
